Does the offence description shown on PNC match the one that would appear on the basic DBO?

Yes. I think the offence name might not appear in the disposal history section at the end of the subject access request document, but it should appear against the arrest record.

does the offence disappear automatically from the DBO once spent or is this a manual process i.e. it can take months for it to be removed post spent date

The conviction will automatically disappear from basic DBS checks processed after it becomes spent, as the rules will tell the DBS system not to disclose it. There is no need to apply.
